INSTRUCTIONS

i)	The society reserves all rights to accept or reject the application.
ii)	No reason shall be given for any application rejected by the society
iii)	No application for membership will be accepted unless it is complete in all respects, Proposed and Seconded by existing member of the A.I.O.S. and accompanied by  Demand Draft for the following amount in favour of "All India Ophthalmological Society" on any Bank in Delhi/New Delhi.

Annual Membership	Rs.500.00		Rs.500.00

Effective 1/4/2003 there will henceforth be no enrollment of member on payment of Annual Membership fee.

There will be only Life Membership on one time Payment of Rs. 4900/- which includes Rs. 400/- as admission fee and Rs. 4500/- as Life Membership Fee."

iv)	Payment should invariably be made through Bank Draft.
v)	Kindly fill the computer card completely.
vi)	Every New Member is entitled to receive the Society's Journal and Annual Proceedings free of charge provided subscription is paid up to date.
vii)	Every New member will be provisionally admitted initially and shall be declared to have become a full member only after formal ratification by the General Body and issue of Ratification order by the Society. Only then he or she will be eligible to vote, propose or contest for any Election of the Society.
viii)	Application  for the Membership along with the Bank Draft for the subscription and admission fee should be addresed to Dr. Prof. (Dr.) Rajvardhan Azad, Hony. General Secretary, All India Ophthalmological Society, Dr. R.P. Centre for Ophthalmic Sciences, A.I.I.M.S., Ansari Nagar, New Delhi - 110 029.
ix)	One Stamp/License Size coloured photograph is to be pasted on the form in the space provided and two Stamp/License size coloured photographs and specimen signatures (in duplicate) are required  to be sent along with this form for issue of Laminated photo Identity card after the Membership is ratified.
